For consideration of your matter, you must include:

• Your email address and a phone number;
• The details of your situation-this means  dates of events and a general explanation of the actions that concern you;
• If your case is already in progress in a court, provide the case number and court;  
• For medical cases, it is extremely helpful if you include the operative or procedure reports for which you have concerns, and any follow-up discharge paperwork; and,
• Attach any documents you have that support your concerns.

Mary Schultz does her best to respond to all emails that are in conformance with the above. If you do not receive a response from her, it may be due to deficiencies in the above process, or to ongoing case load. Please note that our staff intake position has been retired because of the backlog and direct email is the process forward.
