3) A summary of your primary concern- what do you think happened to you or to a family member that was wrong or unlawful?
4) The dates and details of your situation-this means providing the critical dates of events you believe are at issue.
5) What are your damage from these actions?
6) If your case is already in progress in a court, what is the case number and court?
7) Attach documents that can help assess your concern. For medical cases, this would include the operative or admission/discharge reports. For employment cases, including termination cases, any complaints made, investigations and outcomes.
